Share a Smile’s services are referral-only. Unfortunately, due to the demand for our services, we are unable to take patients who are not referred from a Utah County case manager.
Patients must be actively in services with a poverty-based agency in Utah County in order to qualify for Share a Smile.
Here is a list of agencies we are able to accept referrals from:

- WATCH Program
- Ability First
- Utah County Substance Abuse
- Vocational Rehabilitation
- House of Hope
- Foothills
- Turning Point
- Kids on the Move
- Vet Center
- Center for Women and Children in Crisis
- Alpine Recovery
- Chainbreakers
- International Rescue Committee (Utah County Placements only)
- Catholic Community Services (Utah County Placements only)
- Utah Valley Refugee Action Network (Utah County Placements only)
If you reside in Utah County and are currently active in services with any of these agencies, talk with your case manager to be put on their referral list. When our enrollment opens, we will reach out to each of these agencies for a list of qualified individuals.
